    Please read the following carefully and select the Biometric Screening Document that applies to YOU:

     

    A. If you are a New Hire, or are making a Family Status Change, use the following documents: 

     

     

    B. If you did not select the BPS Health Plan for 2017, and are selecting the BPS Health Plan for 2018, use the following documents:

     

     

    C. If you currently have the BPS Health Plan and will be enrolling again in 2018, but did not complete the biometric screening conducted at your site or have the opportunity to visit your doctor, you can visit a BPS Well-Care Center, use the following documents:

     

     

    D. If you wish to visit your doctor to complete the biometric screening, you will need to print out and take the BPS Wellness Activity/Biometric Form to your appointment:

      If you have changed your address, you must login to MIC (My Information Center) and change you address there so that you may continue to receive important information sent by our insurance vendors.  To access MIC, go to the District homepage, hover over STAFF from the top menu, then select "My Information Center".  The system will prompt you with further directions to log in to MIC.
