Enrollment check list
Hillside is using an Online Enrollment Service which will allow parents and guardians the ability to start the enrollment process online by filling out the enrollment form.
Both online and the paper processes require parents/guardians to drop off additional paperwork to the school.
When dropping off the additional paper work please have the following available for the school enrollment staff:
- Name, address, phone and fax number of most recently attended school
- Photo identification of parent/guardian enrolling the student
- Address verification (example: rental agreement or utility bill)
- Legal name verification for student & proof of birth date (Show copy of birth certificate, U.S.Passport, DSHS, or medical records)
*Students with life threatening medical conditions must have the appropriate care plans and medication available prior to attending school.
