*7^th graders less than 13 years of age who have not received a previous dose of varicella vaccine must receive one dose.

*7^th graders 13 years of age and older who have not received a previous dose of varicella vaccine must receive 2 doses separated by at least 28 days.

*A reliable history of disease from a physician/health care provider may be accepted in lieu of vaccine.
