*3 doses of polio vaccine, with at least one dose administered on or after the child's 4th birthday. These 3 doses are required for all students, Kindergarten through 12th grade.

*A child, who has received 4 or more doses of polio vaccine, does not have to receive a dose of polio vaccine after the 4th birthday.

 

Diphtheria/ Tetanus/ Pertussis

*For Kindergarten through 12th grade at least one dose of the DTaP, Tdap, DTP, DT, or Td must have been administered on or after the child's 4th birthday. 3 doses are required for all students, 1st through 12th grade.

*Kindergarten students must receive 4 doses.

 

Measles

(M, M/R, M/M/R, MMRV)

*The first dose must have been administered on or after the child's 1st birthday. The second dose must have been administered at least 28 days after the first dose. Two doses are required for

all students Kindergarten through 12th grade.

 

Rubella

Mumps

Hepatitis B

*Students 11-15 years of age may use an approved alternative two-dose schedule.

 

Varicella

(chickenpox)

*One dose required for all kindergarten students given on or after the 1st birthday.

*7th graders less than 13 years of age who have not received a previous dose of varicella vaccine must receive one dose.

*7th graders 13 years of age and older who have not received a previous dose of varicella vaccine must receive 2 doses separated by at least 28 days.

*A reliable history of disease from a physician/health care provider may be accepted in lieu of vaccine.
