Current health records mandatory for Back to School

ELGIN – All students are required to return to their schools on Wednesday, August 17 with up-to-date health records, including physicals and immunization records. Students starting pre-school, kindergarten, sixth, ninth and twelfth grades must meet specific health requirements in order to attend classes on the first day of school.

 

School District U-46 leaders encourage families to schedule an appointment with their child’s primary doctor as early as possible to ensure compliance by the start of school. Greater Elgin Family Care Center will offer expanded hours to accommodate U-46 families. Appointments can be made by calling 847-608-1344.

 

“Regular physical examinations ensure children remain in good health and are ready to learn,” said U-46 CEO Tony Sanders. “Immunizations allow children to be protected and help protect each other.”

 

In recent years, Illinois schools have seen breakouts of various illnesses, including measles and whooping cough. One contributor to the spread of both diseases is a portion of children going unvaccinated.

 

Students will not be allowed to attend classes if they do not have the following requirements on file by the first day of school, August 17. Schedules will be held for seventh through twelfth graders until proof of meeting all health requirements is submitted. The requirements are as follows:

 

 

Physical Exam: Dated after 8/17/2015. Must include diabetes screening/body mass index health history and parent signature.

Preschool/

DPT: 4 doses

Early Childhood

Polio (IPV): 3 doses

 

Hib: 1 dose after 15 months (or completed series by 12 months)

 

MMR: 1 dose (after 12 months)

 

Varicella: 1 dose (after 12 months)

 

Hepatitis B: 3 doses (third dose after six months of age)

 

Pneumococcal: Primary series or proof of 1 dose after 24 months

 

Lead screening/testing: Testing required for students residing in 60120 zip code

 

Mantoux test for Tuberculosis: Recommended

 

 

Physical Exam: Dated after 8/17/2015. Must include diabetes screening/body mass index health history and parent signature.

Kindergarten and students entering Illinois for the first time

DPT: 4 doses (last one after 4th birthday)

 

Polio (IPV): 4 doses (last one after 4th birthday)

 

MMR: 2 doses (after 12 months)

 

Varicella: 2 doses (after 12 months)

 

Lead screening/testing: Testing required for students residing in 60120 zip code

 

Mantoux test for Tuberculosis: Recommended

 

Vision Exam: Completed by optometrist, ophthalmologist or physician who provides complete eye exams. Due by 10/15/2016.

 

Dental Exam: Completed by licensed dentist. Due by 5/15/2017.

 

 

Physical Exam: Dated after 8/17/2015. Must include diabetes screening/body mass index health history and parent signature.

Sixth Grade

Tdap: 1 dose (must be after age 7)

 

MMR: 2 doses (after 12 months)

 

Varicella: 2 doses (after 12 months)

 

Meningococcal: One dose (on or after the 10th birthday).

 

Hepatitis B: 3 doses

 

Mantoux test for Tuberculosis: Recommended

 

Dental Exam: Completed by licensed dentist. Due by 5/15/2017.

Ninth Grade

Tdap: 1 dose (must be after age 7)

 

MMR: 2 doses (after 12 months)

 

Varicella: 2 doses (after 12 months)

Twelfth Grade

Meningococcal: 2 doses (First dose received on or after the 10th birthday; second dose at least eight weeks after the first dose) or 1 dose (if the first dose was received at 16 years of age or older.)

 

Students who are new to the District and do not have the required health exams by the first day of school will get a two-week grace period to provide proof of compliance. Children who are homeless will not be turned away from school for missing health records.

 

Submit all health information by Monday, August 1 to the Educational Services Center, 355 E. Chicago St. in Elgin or fax it to 847-888-5320. Forms that are completed after Tuesday, August 2 need to be delivered to each student’s school.
