Please Read the Important information below:

 

To become eligible for participation in Catholic Memorial athletics, students must have a current physical (form A or form B depending on physical date) on file in the Athletic office and parents/legal guardians must  complete the Student Athletic Forms mailed to you shortly, to be in compliance with WIAA regulations. 

 

Step 1:

Submit Proof of Physical Exam (either A or B)

A.)  WIAA Pre-Participation Physical Evaluation (Form A)

      Special Note:  All Freshman and New Incoming 

      Students need to complete and return Form A.

B.)  WIAA Alternate Year Athletic Permit Card (Form B)

 

HOW DO I KNOW IF I NEED A PRE-PARTICIPATION PHYSICAL EVALUATION OR AN ALTERNATE YEAR PERMIT CARD.

 

If the date of the student’s last physical exam was before April, 1, 2016, the student is then required to have a Pre-Participation Physical exam (form A). 

 

If the date of the student’s last pre-participation physical exam was on or after April 1, 2016, the student is required to complete the WIAA Alternate Year Permit Card (Form B).  Physical Forms are to be turned into the MAIN OFFICE.

 

Verify your student’s physical date on the CMH family database located on the home page of edline.

After 4/1/16

2017-2018

After 4/1/17

2017-2018 & 2018-2019

 

Step 2:

Student – athletes should not have any outstanding financial obligations owed to CMH at the time of registration. Participation fees will be invoiced. Payments are to be made in the Business Office.

 

Step 3:

Note due to the system change, all the forms below will be part of your back to school packet.  The forms will be mailed the end of July. 

  • Parental Insurance Waiver
  • WIAA Concussion Form
  • WIAA Requirements for Eligibility
  • Activity Athletic Code of Conduct

 

**Please do not give physical forms or payment to coaches.
